Upload the following items with your financial report:
- Equipment Inventory – an Excel spreadsheet is available for your use and must be included. Michigan Fitness Foundation (MFF) defines non-capital equipment as “anything that plugs in” or “anything that recharges by plugging in” that costs less than $5,000 per piece.
Clicking the ‘Submit Form’ button at the end of this report will submit this portion of the final report to Michigan Fitness Foundation. You will receive email confirmation of your submission.
