Photo/Video/Audio Release

As part of partners’ contractual obligation to protect privacy, particularly in regard to minors, photographs, video, or audio recordings that are to be made public must have a release completed for each person who is captured and recognizable in photographs or video/audio recordings. Partners can follow their own policies for photos or video/audio recordings that to be used only for internal purposes. Procedure:

  1. Parents or guardians must sign the multimedia release prior to their children being photographed or captured on video or audio.
  2. If it is known ahead of time that photos, video, or audio recordings will take place (for a nutrition-based class for children, as an example), multimedia releases can be completed at the beginning of the class or program year. NOTE: many schools have releases signed by parents at the beginning of the school year and this release may be sufficient for your needs.
  3. A photo/video/audio release form for models (people) is available in both Word and PDF formats below
